Firearms Applications

For patients requesting completion of a firearms application and a medical fitness form to be signed, our policy is that as GP’s are not contracted on the NHS to complete these reports and they have the right to decline to provide the service.  Our GP’s will not sign these applications on grounds of conscientious objection and also because it falls outside the remit of their medical expertise. 

However, it is possible for this assessment to be done privately by Dr Oliver Denton who is a private GP based at Daleswood Health, Barn House, Barston Lane, Solihull B92 0JJ.  A half hour appointment can be arranged by phoning - 01675489489.  We are informed the fee for making the assessment of fitness and signed paperwork if this is appropriate is £125.00. 

Dr Denton will require access to the GP medical record of the applicant and it is responsibility of the applicant to let Hampton Surgery know by letter or email hampton.surgery1@nhs.net when the assessment appointment is so that their full medical record goes direct from Hampton Surgery with their permission to Dr Denton at Daleswood Health, or alternatively complete a consent form for release of medical records via Daleswood Health. 

For information governance to be followed Dr Denton must receive the applicant’s full medical record direct from the GP practice and not via the applicant. 

All fees to Daleswood Health are the responsibility of the applicant.
