For patients requesting completion of a firearms application and medical fitness form to be signed, Hampton Surgeries policy is that as GP’s are not contracted on the NHS to complete these reports and they have the right to decline to provide the service. NHS GP’s do not sign these applications on grounds of conscientious objection and also because it falls outside the remit of their medical expertise.
However, it is possible for this assessment to be done privately. You would need to source local and national services to suit your requirements.
The assessing Doctor will require access to the GP medical record of the applicant and it is responsibility of the applicant to let Hampton Surgery know by letter or email hampton.surgery1@nhs.net when the assessment appointment is so that their full medical record goes direct from Hampton Surgery with their permission to the assessing Doctor, alternatively complete a consent form for release of medical records to the assessing Doctor.
For information governance to be followed the assessing Doctor must receive the applicant’s full medical record direct from the GP practice and not via the applicant.
